How jaundice is manifested in an infant and what to do with it

How does jaundice manifest in a baby and what to do with it?

What is bilirubin and where does it come from?

Bilirubin in a newborn is exactly the same substance that is formed in the body of an adult. Its source is the breakdown products of proteins, in particular, hemoglobin, as well as other substances that are involved in the process of human oxygen metabolism.

Blood in the tubeDue to the continuity of metabolic processes, bilirubin is always contained in the blood plasma. Its norm for an adult is 8.5–20.5 μmol per liter, but in the first days of life in a newborn the rate can rise to 205 μmol per liter. What is the reason for this phenomenon?

High bilirubin in a newborn is a consequence of the fact that during the development of the fetus in the mother’s body he cannot breathe naturally and receives oxygen exclusively through the blood.

For this, erythrocytes of the maternal body are used, which contain a large amount of hemoglobin.As soon as the connection between the blood systems is interrupted, most of the red blood cells die, because they are no longer necessary, and the baby’s body begins to produce its own blood. That is why the rate of bilirubin in his body is so much greater than that of an adult.

Pretty pink-cheeked babyIn the normal state, bilirubin is excreted with the natural waste products of the body after it has undergone binding by enzymes. However, the newborn enzymes are not yet being developed, or are not produced in sufficient quantities. That is why the norm established for an adult is not valid for a baby. Getting rid of excess bilirubin begins only after some time, when all metabolic processes begin to work at full capacity.

Jaundice in the newborn and its causes

Elevated bilirubin in newborns can manifest itself in a change in the state of the body, in particular, the skin color becomes reddish-yellow, the baby is constantly worried and often cries. Usually, this condition does not require additional treatment, as it is completely eliminated on its own within the first few days.However, in some children the rate may be exceeded over a long period of time.

The main reasons for this are:

  • pathological processes in the body;
  • lack of oxygen during fetal development for various reasons;
  • maternal diseases during pregnancy
If bilirubin in newborns is not removed at the appropriate time, it can lead to toxic damage to the nervous system.

By the age of six months, such children begin to lag significantly behind their peers. In the future, the forecast is disappointing - a significant limitation of physical and mental abilities. In addition, increased bilirubin can be displayed on the efficiency of the digestive system, disrupting the metabolism.

Phototherapy procedures for a newbornSpecific treatment in this case is not prescribed, since it will not give a significant positive effect. Instead, phototherapy is indicated for babies with elevated bilirubin, that is, exposure to sunlight. As a result of this exposure, the enzyme is converted to lumirubin, which has no toxic effect on the human body and is eliminated within 12 hours.

In addition, the rate can be quickly achieved when an early attachment to the chest is applied. The accelerated output of the primary feces helps to eliminate excess bilirubin.

However, it also happens that bilirubin in the newborn is elevated as a result of the influence of pathological processes. Among them are:

  • genetic abnormalities affecting the liver;
  • infections and mechanical damage to the liver, disrupting its normal activities;
  • intestinal obstruction;
  • endocrine disruption;
  • incompatibility of the Rh factor of the mother and child.

Such pathologies are quite rare and occur in no more than 2% of cases, but you need to know about their existence. To restore the normal content of bilirubin in the blood, it is necessary to apply both the above recommendations and the specific treatment prescribed by the neonatologist.
